1. Are all the investments you have made through your Representative listed on the account statements that you receive from our firm, including statements from Mutual Fund and Annuity companies?

2. Have you had any operational problems with the firm (missing dividends, interest payments, transfer problems, etc.) that took an unreasonable length of time to resolve?

3. Do you receive trade confirmations from us in a timely manner?

4. Are there any additional questions you have or comments you may wish to provide?

Sending Checks & Stock:
For purchases made within your account, please make all checks payable to ‘National Financial Services LLC’ and be sure to include your account number on your check. Stock certificates must be endorsed by writing ‘National Financial Services LLC’ between the words ‘appoint’ and ‘attorney’ either on an accompanying Stock Power (preferred) or on the back of each certificate. All owners must sign the Stock Power or each certificate exactly as registered.
